To register your child/children, please fill in the form below. 

It is important that you enter your details accurately.

We require students to be a minimum age of  7 years old due to our experiential and inquiry-based pedagogy.

 

Each submission is for one child.

 

If you have more than one child to register, please complete another submission.

Our termly school fees per child will be $130.00, which consist of :
$120.00 - Term Fees
$10.00 - Stationery Fees

For 2023, we will require all students to pay their school fees before the start of each term in order to secure a place in the school.

SECTION 1: STUDENT DETAILS

Is the student currently attending any VELS approved language program ?
Does the student have any allergy / allergies (eg, Chemicals, plants, food items etc..)
Do you have an Ambulance Cover and/or private health insurance ?
Do you acknowledge and agree for us to call an ambulance in case of an emergency and when the parent/guardian of the students is not contactable?

SECTION 2: PARENT DETAILS

Are you residing at the same address as the Student ?

CONSENT FOR USE OF IMAGES/VIDEOS

I give permission for images in which my child/ward and/or I, appear to be used by Dar Nafisa Academy INC for publications and public relations activities. In particular, I note that this may include use in print and electronic media, including the Internet and official social media platforms.

 

I have read and understand this notice, and consent to the collection, use and disclosure of my image as outlined. Please leave the box unchecked if you do not wish to give consent to the above.

ACCEPTANCE OF TERMS AND CONDITIONS

I, hereby declare that all the information entered in this page are true and correct to the best of my knowledge. By ticking the box below, I agree that Dar Nafisa Academy INC will not be held responsible to any false or incorrect information entered in any part of this registration process.  In times of medical emergency the school will attempt to contact the emergency number provided. Should the parent/guardian/caretaker not be contactable, we will proceed to contact the relevant medical authorities at your cost.
